Job Title: Psychiatric Care Coordinator

About the Position

We are seeking a detail-oriented and compassionate Psychiatric Care Coordinator to join our team at High Country Behavioral Health in Douglas, WY. As a key member of our residential facility, Solutions House, you will play a vital role in coordinating patient care and services.

This is a part-time and full-time position available, with shifts ranging from 12+ hours, including days, nights, and weekends. You must be available to attend orientation and training sessions.

Responsibilities
  • Coordinate patient care services, including scheduling appointments and managing medication regimens.
  • Communicate with healthcare providers to share patient information and treatment plans.
  • Assist patients with daily living activities, such as personal care and meal preparation.
  • Develop and implement individualized treatment plans.
  • Monitor patient progress and make recommendations for continued care.

Salary Range: $42,000 - $52,000 per year, depending on experience.

Requirements
  • High School Diploma or Equivalent.
  • Previous experience working in healthcare or social services preferred.
  • Ability to work effectively in a fast-paced environment.
  • Strong communication and organizational skills.
  • CPR/First Aid certification (or willingness to obtain).
