Job Title: MDS Coordinator/Case Manager

We are seeking a highly skilled and experienced MDS Coordinator/Case Manager to join our team at Meadows of Cadiz. As a key member of our interdisciplinary team, you will play a vital role in ensuring the highest quality of care for our residents.

Responsibilities:
  • Develop and implement comprehensive care plans that meet the unique needs of each resident
  • Conduct thorough assessments of resident needs and develop plans to address those needs
  • Collaborate with the interdisciplinary team to ensure that all aspects of resident care are addressed
  • Ensure that all necessary documentation is completed accurately and in a timely manner
  • Participate in the development and implementation of policies and procedures related to resident care
  • Provide education and training to staff on MDS 3.0 and care planning
  • Lead the daily PPS and Utilization Review Meeting, ensuring all applicable IDT members are present and participate
  • Lead the weekly Triple Check IDT Meeting, ensuring all applicable team members are present and participate
  • Lead the weekly Case Mix Review Meeting
  • Participate in resident admission referral process
  • Case Management - Provide resident clinical update and re-certification information to Managed Care Providers Case Managers in a timely manner
  • Participate in the weekly Standards of Care Meeting
  • Lead all scheduled Resident Care Conferences/Care Plan Review meetings
  • Coordinate the interdisciplinary assessment process for all residents of the facility
  • Educate peers on MDS 3.0, CAAs, and Care Plans
  • Attend in-service education programs in order to meet facility educational requirements
  • Be familiar with Standard Precautions, Exposure Control Plan, Fire Drill and Evacuation Procedures and know how to use the information
  • Maintain confidentiality of resident and facility records/information
  • Protect residents from neglect, mistreatment, and abuse
  • Protect the personal property of the residents of the facility
  • Others as directed by the supervisor or administrator
Requirements:
  • High school diploma or equivalent required; associate's or bachelor's degree in a related field preferred
  • Minimum of 2 years of experience in a long-term care setting, preferably in a case management or MDS coordinator role
  • Strong knowledge of MDS 3.0 and care planning principles
  • Excellent communication and interpersonal skills
  • Ability to work effectively in a team environment
  • Strong organizational and time management skills
  • Ability to maintain confidentiality and handle sensitive information
