Job Summary:

We are seeking a skilled RN Case Manager to join our team at Divine Home Care and Hospice. As a RN Case Manager, you will provide case management services to a team of clients, utilizing nursing theory and process, and in accordance with the State Nurse Practice Act.

Key Responsibilities:

  • Provides case management services to a team of clients
  • Utilizes nursing theory and process to communicate and document observations and assessments
  • Reports to the Administrator, Director of Nursing, and Branch Manager
  • Directs a team of RNs, LPNs, Office Personnel, Home Health Aides, and Personal Care Attendants


Requirements:

  • RN License (Required)
  • Driver's License (Required)
  • 1 year of nurse management experience (Preferred)
  • High school or equivalent (Preferred)
