Job Title: Social Services Specialist I

Sea Mar Community Health Centers is seeking a highly motivated and compassionate Social Services Specialist I to join our team. As a key member of our MSS Department, you will play a vital role in providing comprehensive integrated clinical psychosocial assessments, developing intervention plans, and providing brief treatment to clients in need.

Job Summary:

The Social Services Specialist I will work closely with our interdisciplinary team to provide whole-person care to clients, addressing their physical, emotional, and social needs. This role requires a strong understanding of community resources, evidence-based screening tools, and the ability to work effectively with diverse populations.

Responsibilities:
  • Conduct initial screenings and comprehensive integrated clinical psychosocial assessments to identify client needs and develop intervention plans.
  • Provide brief treatment and transition services for long-term counseling and treatment.
  • Consult with the interdisciplinary team to ensure seamless care coordination and address client needs.
  • Develop and maintain relationships with community resources and safety-net providers to facilitate whole-person care.
  • Utilize evidence-based screening tools to inform level of care interventions and referrals.
Requirements:
  • Bachelor's degree from an accredited university and current Certified Counselor through the State of Washington Department of Health or Chemical Dependency Counselor.
  • Experience working with underserved populations, including those with limited English proficiency, homelessness, and migrant and seasonal workers.
  • Knowledge of community resources and safety-net providers to facilitate whole-person care.
  • Ability to work effectively in a fast-paced environment and prioritize multiple tasks and responsibilities.
