Key Responsibilities- Provide peer support and guidance to individuals with mental health conditions and their families.
- Assist in the development and implementation of comprehensive treatment plans.
- Collaborate with healthcare providers, community agencies, and other stakeholders to ensure seamless care coordination.
- Recruit and support consumers, parents, and family members to participate in all phases of our operations.
- Participate in treatment planning with consumers and families who request support.
- Work with community agencies and on behalf of consumers and families to facilitate access to and transition between services.
- Provide feedback about treatment planning development based on interactions with other consumers and families.
- Advocate on behalf of consumers and families.
- Provide peer support to consumers and family members at critical points in their treatment process.
- Serve as a liaison between UnitedHealth Group, the Health Plan, and those who use our services.
- Active and current Peer Recovery Specialist or Peer Support Specialist certification in the state of Kentucky OR ability to obtain certification within 12 months of employment.
- 4+ years of case management experience.
- 1+ years of experience with community resources.
- 1+ years of experience with demonstrated understanding of the principles of recovery and resiliency and lived experience.
- 1+ years of experience with the Medicaid population.
- Proven intermediate level of proficiency with Microsoft Office (Word, Excel, and Outlook) and ability to access multiple web applications.
- Reside within a commutable distance to Lexington and surrounding communities in Kentucky.
- Current and non-restricted state of Kentucky driver's license and state-required insurance and access to reliable transportation with the ability to travel 75% of the time within 60 miles of territory assigned.
- Proven access to a designated quiet workspace in your home (separated from non-workspace areas) with the ability to secure Protected Health Information (PHI).
- Live in a location that can receive a UnitedHealth Group approved high-speed internet connection or leverage an existing high-speed internet service.
- Proven bilingual in (English/Spanish).
- Demonstrated ability to facilitate training events.
