About the Position:
The NYC Health + Hospitals organization is seeking a highly skilled and compassionate Mental Health Support Professional to join their team. This individual will be responsible for providing critical mental health support services to patients in correctional facilities, focusing on safety planning, crisis intervention, and post-release reintegration. Main Responsibilities:
- Assess patients' mental health needs and develop targeted interventions to address their specific challenges
- Collaborate with multidisciplinary teams to deliver holistic care and promote positive patient outcomes
- Develop and implement safety plans to prevent harm and ensure patient safety
- Provide crisis intervention and stabilization services to patients experiencing acute mental health episodes
- Foster partnerships with community-based organizations to enhance resource access and patient support networks

Requirements:
- Master's degree in Social Work, Counseling, or a related field
- At least two years of experience working in a mental health capacity, preferably in a correctional setting
- Strong understanding of trauma-informed care principles and practices
- Excellent communication and interpersonal skills, with ability to build trust and rapport with patients and colleagues
- Ability to navigate complex systems and prioritize competing demands while maintaining a patient-centered focus
