About the Role

We are seeking a skilled Healthcare Coordinator to join our team at UnitedHealthcare. As a key member of our team, you will play a vital role in ensuring the delivery of high-quality, patient-centered care.

Key Responsibilities
  • Assess and Plan Care: Conduct thorough assessments of patient needs and develop individualized care plans to ensure the most effective and efficient delivery of care.
  • Coordinate Care Services: Collaborate with healthcare providers, community resources, and social services to coordinate care services and ensure seamless transitions for patients.
  • Advocate for Patients: Act as a patient advocate, communicating with healthcare providers and family members to ensure that patient needs and preferences are respected and met.
  • Manage Care Plans: Monitor and update care plans as needed, ensuring that patients receive the most effective and efficient care possible.
Requirements
  • Current RN License: Possess a current, unrestricted RN license in the state of Hawaii.
  • Clinical Experience: Have a minimum of 2 years of clinical experience, with a focus on long-term care, home health, hospice, public health, or assisted living.
  • Technical Skills: Possess intermediate-level proficiency with MS Word, Excel, and Outlook, as well as the ability to travel locally to visit patients in their homes and other settings.
Preferred Qualifications
  • Bachelor's Degree: Possess a bachelor's degree or higher in a related field.
  • CCM Certification: Hold a CCM (Certified Case Manager) certification.
  • Experience with Electronic Charting: Have experience with electronic charting and arranging community resources.
