As a member of our multidisciplinary healthcare team, you will be responsible for making visits to clients, performing diagnostic tests, and developing individualized care plans.
Responsibilities:
- Client Visits: Make regular visits to clients in their homes to assess their physical needs and provide personalized care.
- Diagnostic Testing: Perform diagnostic tests and measurements to assess clients' mobility, strength, and balance.
- Care Planning: Develop and implement individualized care plans to meet clients' physical therapy needs.
- Education and Training: Educate clients and their families on rehabilitative care and activities to promote independent living.
- Documentation: Accurately document observations, interventions, and evaluations related to client care management.
Qualifications:
- Licensure: Current license as a Physical Therapist in Massachusetts.
- Experience: Minimum of one year of recent clinical experience as a licensed Physical Therapist.
