Job Summary

As a Wound Care Coordinator at Optalis Healthcare, you will be responsible for overseeing the overall wound care and skin concerns within our facilities. You will lead weekly wound team rounds and processes, including rounds, measurements, interdisciplinary care plan review, treatment recommendations, physician orders, and required documentation.

Key Responsibilities
  • Work with the ancillary wound care team to coordinate care for residents with skin concerns.
  • Obtain treatment orders as needed and assist with modifications to orders if necessary.
  • Assess new admissions to verify skin assessments are correct.
  • Review all resident charts weekly to ensure timely completion of weekly skin checks.
  • Update wound care plans and spot check dressing changes.
  • Hold weekly skin and wound meetings with the IDT team.
  • Assess and perform treatment for residents with stage IIIs, stage IVs, and IHA.
Requirements
  • Demonstrated knowledge of payor sources, referral sources, and community resources.
  • Excellent verbal and written communication skills.
  • Excellent interpersonal and customer service skills.
Education and Experience

RN (preferred) or LPN with experience in wound care and skin management.

Physical Requirements

Prolonged periods of sitting at a desk and working on a computer. Ability to lift up to 15 pounds at times.
