Job Summary

We are seeking a highly skilled HCBS Care Coordinator to join our team at UnitedHealthcare. As a Care Coordinator, you will play a critical role in ensuring that individuals receive the necessary services and support to live independently in their communities.

Key Responsibilities
  • Assess and develop individualized care plans that address the unique needs and goals of each person
  • Coordinate with healthcare providers, community resources, and family members to ensure seamless care transitions
  • Identify and connect individuals with community-based services and support, including financial, psychosocial, and informal supports
  • Monitor and evaluate the effectiveness of care plans, making adjustments as needed to ensure optimal outcomes
  • Collaborate with interdisciplinary teams to address social determinants of health and promote health equity
Requirements
  • Bachelor's degree in social work, rehabilitation, nursing, psychology, special education, gerontology, or related human services area
  • 1+ years of experience working with individuals with long-term care needs
  • Intermediate level of computer proficiency, including Microsoft Outlook, Teams, and web applications
  • Ability to travel 75% of the time, with reliable transportation and a valid driver's license
Preferred Qualifications
  • Licensed Social Worker or clinical degree
  • Experience working with individuals receiving services on home and community-based waivers in Kansas
  • Background in managing populations with complex medical or behavioral needs
