Clinical Management Consultants is seeking a highly skilled MDS Nurse Coordinator to join our team. As a key member of our clinical operations team, the MDS Nurse Coordinator will play a critical role in ensuring the delivery of high-quality patient care services.
Key Responsibilities
- Coordinate patient care activities, including the implementation of policies and procedures, patient care delivery and processes, and evaluation of patient care activities and reporting findings to the Director.
- Ensure the completion of interdisciplinary patient care plans for patients admitted to the Transitional Care Unit.
- Collaborate with healthcare professionals to develop and implement effective patient care strategies.
- Monitor and analyze patient care data to identify trends and areas for improvement.
- Develop and implement quality improvement initiatives to enhance patient care services.
Requirements
- Current licensure as a Registered Nurse (RN) in the state of New Mexico.
- Minimum of 2 years of experience in a clinical setting, preferably in a leadership or coordination role.
- Strong knowledge of patient care policies and procedures.
- Excellent communication and interpersonal skills.
- Ability to work effectively in a fast-paced environment.
