Salary: $65,000 - $85,000 per year, commensurate with experience.

Job Description:

We are seeking a detail-oriented Nurse Care Coordinator to join our team. As a Licensed Practical Nurse, you will be responsible for coordinating care services for residents, working closely with healthcare teams to ensure seamless transitions, and monitoring progress towards care goals. Your excellent organizational and communication skills will enable you to succeed in this role, collaborating with multidisciplinary teams to provide comprehensive support to our residents.

Required Skills and Qualifications:

* Current NYS LPN License
* One year of healthcare experience working with adults or the aging population in a clinical setting
* Strong analytical and problem-solving skills
* Proficiency in electronic medical records (EMR) systems
