Job Summary

Fairfax Location & Community Involvement - The Community Outreach Worker plays a vital role in promoting healthy living and empowering individuals to take control of their health and lives. This position requires a deep understanding of the experience, language, and culture of the populations served, with a focus on providing culturally appropriate health education and information.

Key Responsibilities
  • Provides direct support and advocacy for patients experiencing community violence, including screening, assessment, crisis intervention, and case management.
  • Facilitates communication and networking amongst service providers to ensure seamless service connections for patients.
  • Provides accompaniment for victims of community violence with Fairfax Healthcare Systems as needed.
  • Offers ongoing intensive case management to individuals, connecting them with resources for economic, food, housing, educational, social, and medical support.
  • Assists in administering and tracking crisis funds for victims of intentional violence.
  • Participates in community outreach through meetings, conferences, committees, and task forces focused on violence prevention efforts.
  • Demonstrates knowledge of growth and development principles and assesses patient data to provide tailored care.
  • Performs additional duties as assigned.
Requirements
  • Associate Degree or HS Diploma/GED and 2 years of experience.
  • Strong understanding of cultural competence and community outreach principles.
  • Excellent communication and interpersonal skills.
  • Ability to work in a fast-paced environment and prioritize multiple tasks.
