LVN Charge Nurse Leader

We are seeking a highly skilled and compassionate LVN Charge Nurse to join our team at Regency Integrated Health Services.
Job Summary:The LVN Charge Nurse will play a vital role in ensuring the delivery of high-quality care to our residents. This individual will be responsible for overseeing the daily operations of the nursing unit, collaborating with the interdisciplinary team, and promoting a culture of excellence and patient-centered care.
Key Responsibilities:- Assist in the development and implementation of individualized care plans that promote the highest practicable physical, mental, and psychological well-being of each resident.
- Conduct regular rounds to assess resident condition, document changes, and report to the interdisciplinary team.
- Administer medications as prescribed and ensure the implementation of care plans as directed.
- Collaborate with the physician to conduct rounds and record visits in clinical records.
- Perform narcotic reconciliation and identify significant changes in resident condition, taking necessary action per facility policies and procedures.
- Oversee and monitor aides, ensuring they provide high-quality care and support to residents.
- Initiate and complete Incident and Accident Reports per facility policy.
- Discharge residents per facility policy and complete Nursing Assessments for Physician Calls prior to calling the physician on any change in resident condition.
- Communicate with resident's point of contact regarding status updates or changes in condition.
- Graduate of an accredited LVN program.
- Active Licensed Vocational Nurse in the state of Texas in good standing.
- Experience working in a skilled nursing facility is preferred.
