Job Summary

Kinwell is seeking a highly skilled Clinical Care Registered Nurse to join our team in Pasco, Washington. As a Clinical Care RN, you will provide comprehensive nursing care to a diverse group of patients and families across a lifespan in a community-based setting.

Key Responsibilities
  • Provide patient-centered care, including assessment, planning, implementation, and evaluation of care plans.
  • Collaborate with clinicians and other healthcare team members to coordinate patient care.
  • Triangulate, assess, plan, schedule, coordinate, evaluate, and delegate the delivery of patient-centered nursing care in the primary care setting.
  • Identify at-risk patients, develop care plans, and coordinate care with patient involvement.
  • Standardize tasks and workflows to support all staff and providers working to the top of their licensure.
  • Use technology, such as tele-visits and remote monitoring capabilities, to engage patients and monitor chronic health conditions.
  • Administer vaccinations and medications as appropriate, and perform testing as directed by a provider.
  • Support follow-up patient care gaps, including colonoscopies, mammography, A1c, and other preventive measures.
  • Provide Transitional Care Management (TCM) following an inpatient stay for all identified patients.
  • Work with patients to develop plans to improve general health status.
  • Implement physical and psychological preventive measures.
  • Promote patient self-care responsibility for meeting their own health needs.
  • Maintain concise, accurate, and timely documentation that supports effective, efficient management of the member and meets accreditation, contractual, and legal requirements.
Requirements
  • WA State RN licensure.
  • Three (3) years' experience as a RN in physician's office, family practice, or similar medical setting.
  • BLS required, current AHA Healthcare Provider training (BCLS); ACLS & PALS desired within 6 months of hire.
  • Understanding of Medicare documentation, billing, and reporting requirements associated with Annual Wellness Visits (AWV), Chronic Care Management (CCM), and Transitional Care Management (TCM) services.
  • Knowledge of care continuum (inpatient, ambulatory, post-acute, etc.).
  • Experience with Epic Electronic Medical Record.
  • Bilingual.
