Position Summary:
The Bereavement Support Specialist is responsible for designing, executing, and overseeing a bereavement program that assists families and caregivers of the deceased for up to one year following the loss.
Key Responsibilities:
- Conduct a Bereavement Assessment within five days of patient admission as part of the overall evaluation process.
- Develop and implement a tailored Bereavement Plan of Care based on the assessment, risk factors, and insights from the interdisciplinary team regarding bereavement challenges during patient care and after the death.
- Offer bereavement support to hospice staff managing grief related to their professional responsibilities.
- Educate the interdisciplinary team on bereavement-related issues.
- Facilitate memorial services, grief support groups, community outreach, and provide necessary follow-up communications and visits.
- Connect families with community resources and professional services as needed.
- Commit to personal growth by enhancing bereavement practices through ongoing education, workshops, and active participation in relevant professional organizations.
- Ability to access patient residences, which may not always be wheelchair accessible.
- Physical activities may include walking, sitting, bending, and standing.
- Proficient use of office equipment, including computers and telephones.
- Availability for on-call duties.
- Demonstrated self-confidence and a positive outlook towards personal and team goals.
- Ability to assess and recognize personal strengths and areas for improvement.
- Knowledge of relevant local, state, and federal regulations.
- Strong assessment and problem-solving capabilities.
- Valid driver's license and access to a reliable, insured vehicle.
- Familiarity with spreadsheet software.
- Exceptional written and verbal communication skills.
- Strong organizational and time management abilities.
- Ability to handle sensitive and confidential information with discretion.
- Ordained, commissioned, or credentialed by an organized religious body, with proof of at least one unit of Clinical Pastoral Education (CPE).
- Alternatively, a degree in theology or from a seminary, with completion of at least one unit of CPE from a recognized provider.
- Preferred: Four units of CPE and a letter of endorsement from a hospice organization.
- Comfortable working in an interfaith environment.
- Experience in dealing with death and dying, including support for individuals, families, and caregivers.
- Previous hospice experience is advantageous.
- Open-mindedness towards diverse lifestyles, cultures, beliefs, and values.
- Effective communication skills, both verbal and written.
