About the Role

We are seeking a skilled Speech Language Pathologist to join our team at VNA Health Group. As a Home Health Speech Language Therapist, you will be responsible for providing high-quality speech and language therapy services to patients in their homes or community settings.

Key Responsibilities
  • Assess and evaluate clients for speech and language therapy needs, developing and implementing individualized plans of action in collaboration with physicians.
  • Document all changes in client condition and update care plans and outcomes accordingly.
  • Identify counseling needs and plan appropriate teaching strategies.
  • Monitor quality and appropriateness of services provided, adhering to agency infection control and safety policies.
  • Adhere to the organization's policy regarding absenteeism and appearance.
Requirements
  • Licensed Speech-Language Pathologist with CCC-SLP certification and Master's degree in Speech-Language Pathology.
  • 2-3 years of prior clinical experience and community-based experience preferred.
  • Strong computer skills and effective communication and interpersonal skills.
  • Valid NJ driver's license.
Working Conditions/Physical Demands

Working indoors and outdoors, driving, lifting, sitting, standing, walking, fine motor hand movement, use of phone and tablet for documentation.
