Job Summary

We are seeking a highly skilled and dedicated Observation Unit Case Manager to join our team at Stony Brook Medicine. As a key member of our Care Management Department, you will play a vital role in providing exceptional patient care and ensuring seamless transitions between care settings.

Key Responsibilities
  • Conduct thorough utilization reviews for inpatient and observation cases to ensure accurate placement and optimal care.
  • Collaborate closely with Case Managers, Social Workers, and other healthcare professionals to develop and implement comprehensive care plans.
  • Identify and address potential delays in care, ensuring timely and safe discharges for patients.
  • Review and document cases for potential changes in level of care, working closely with physicians and payers to prevent denials.
  • Communicate effectively with patients, families, and healthcare teams to ensure seamless transitions and optimal outcomes.
  • Assist with coordination of discharge planning, including patient and family education, and preparation of PRI for early referrals.
  • Participate in performance improvement activities, including program development, data collection, and analysis.
Requirements
  • Bachelor's degree or RN license with 5 years of hospital experience.
  • Excellent communication and interpersonal skills, with ability to work effectively with diverse teams.
  • Strong analytical and problem-solving skills, with ability to prioritize and manage multiple cases.
  • Knowledge of regulatory requirements and industry standards.
  • Preferred: CM, UM, Quality, Coding, Risk, or MCG Certification, or working knowledge of MCG or Interqual PRI.

Stony Brook Medicine offers a competitive salary range of $99,665 - $114,484, plus a $3,400 location salary grade. We also offer generous leave, health plans, and state pension benefits.
