Job Summary

Maxim Healthcare Services is seeking a skilled Registered Nurse (RN) or Licensed Practical Nurse (LPN) to provide high-quality patient care in a home healthcare setting. As a key member of our team, you will be responsible for delivering patient-centered care, collaborating with healthcare professionals, and ensuring the best possible outcomes for our patients.

Responsibilities
  • Assessment and Planning: Conduct thorough assessments of patients' physical and psychosocial needs, develop individualized care plans, and prioritize interventions to achieve optimal patient outcomes.
  • Care Delivery: Provide direct patient care, including administering medications, performing wound care, and managing medical equipment, in accordance with established protocols and guidelines.
  • Documentation and Communication: Accurately document patient information, communicate effectively with healthcare teams, and maintain confidentiality in all interactions.
  • Education and Support: Educate patients and their families on disease management, medication adherence, and healthy lifestyle choices, promoting self-advocacy and empowerment.
  • Collaboration and Teamwork: Work collaboratively with interdisciplinary teams, including physicians, therapists, and social workers, to ensure comprehensive patient care and achieve shared goals.
Requirements
  • Certifications and Licenses: Current RN or LPN licensure in the state of practice, BLS certification, and PPD or chest X-ray.
  • Experience and Skills: One year of prior nursing experience, with a focus on home healthcare or a related field, and proficiency in enteral feeds, tracheostomy care, and ventilator management.
