Job Overview:

Illumia Health is seeking a skilled Registered Nurse to join our team as an Afterhours Palliative Care RN in Advanced Illness Management. This role requires providing high-touch palliative care to patients and their families during afterhours, ensuring comfort through preventive and supportive interventions.

Responsibilities:
  • Triage patient needs over the phone until a nurse can provide an in-person visit
  • Respond to crisis calls from patients and families after hours with urgency and compassion
  • Making initial nursing evaluations to determine eligibility for palliative services within 48 hours of referral
  • Identifying physical, psychosocial, emotional, and environmental needs and reassessing as needed
  • Collaborating with interdisciplinary teams to provide patient and family care
Requirements:
  • Associates/Bachelor's degree in Nursing
  • 2 years of prior skilled Registered Nurse experience
  • Demonstrated ability to provide quality patient care
  • Nursing experience in palliative care, oncology, hospice, or geriatric care preferred
