Job Description

Job Summary: We are seeking an experienced Registered Nurse Case Manager to join our team. The ideal candidate will have a strong background in palliative care, excellent communication skills, and the ability to work effectively in a fast-paced environment.

Responsibilities
  • Patient Care: Manage patient care functions after initial admission and assignment to a designated RN Case Manager. Assist in the admission process as requested and interpret policies and procedures for nursing staff and others.
  • Interdisciplinary Team Collaboration: Coordinate and collaborate with interdisciplinary team members for the management of the plan of care on an ongoing basis to meet patient/family needs.
  • Documentation and Communication: Update the Plan of Care on an ongoing basis, keeping other team members current regarding the progress of the patient's physical status and any changing needs for care. Facilitate effective written and verbal communication with patients/families and other healthcare providers.
  • CNA Scheduling and Referrals: Coordinate CNA scheduling needs with Patient Care Coordinator/Area Manager or designee and initiate referrals for other hospice-related care and services.
  • Education and Community Outreach: Provide education on hospice and hospice services for patients/families, other team members, and the community. Participate in community outreach activities to educate the public about Arkansas Hospice services.
  • Rotating On-Call Duty: Rotate on-call duty to provide 24-hour service to patients and families as assigned.
  • Confidentiality and Customer Service: Maintain strict confidentiality at all times and utilize and model excellent customer service skills at all times.
Requirements
  • Education: Graduation from an accredited school of nursing; Bachelor of Science in Nursing (BSN) preferred.
  • Experience: Minimum of one year nursing experience in a hospital, home health agency, hospice, or related program.
  • Licensure: Licensed as a Registered Nurse (RN) in Arkansas.
  • Additional Requirements: Valid Arkansas driver's license, personal automobile, automobile insurance required.
