Job Title: Registered Nurse Case Manager

Interim HealthCare of New Hampshire is seeking a skilled Registered Nurse Case Manager to join our team.

Job Summary:

We are looking for a dedicated and compassionate Registered Nurse to manage a defined caseload of patients, providing direct patient care and comprehensive nursing services to individuals in the community.

Responsibilities:
  • Manage a caseload of patients, providing direct care and coordinating services to meet their needs.
  • Conduct thorough assessments of patients to identify their healthcare needs and develop individualized care plans.
  • Collaborate with healthcare teams to ensure seamless care coordination and communication.
  • Provide education and support to patients and their families to promote health and wellness.
  • Document patient information and outcomes in a timely and accurate manner.
Requirements:
  • Valid nursing license in the state of New Hampshire.
  • 2+ years of experience in skilled care, preferably in a case management role.
  • 1+ year of experience in nursing supervision, preferred.
  • Recent wound care experience.
  • Ability to lift 50 pounds with no restrictions.
  • Current CPR certification.
  • Reliable transportation.
