Job Title: Registered Nurse Case Manager

Job Summary:

We are seeking a skilled Registered Nurse to join our team as a Case Manager. The successful candidate will be responsible for managing a defined caseload of patients, providing direct patient care and comprehensive nursing services to individuals in the community.

Key Responsibilities:

  • Manage a caseload of patients and provide direct care
  • Develop and implement individualized care plans
  • Collaborate with healthcare teams to ensure comprehensive care
  • Provide education and support to patients and families

Requirements:

  • Valid nursing license in the state of NH
  • 2+ years of experience in skilled care preferred
  • 1+ year of experience in nursing supervision preferred
  • Recent wound care experience
  • Ability to lift 50 LBS with no restrictions
  • Current CPR certification
  • Reliable transportation
