Job Summary

UPMC Home Health is seeking a full-time Licensed Practical Nurse (LPN) to join our team in the Mount Washington, Greentree, Crafton, Corliss, Carnegie, and surrounding areas. As an LPN, you will play a vital role in providing high-quality patient care and contributing to the success of our healthcare team.

Key Responsibilities
  • Provide nursing care to patients in their homes, following UPMC LPN Practice Guidelines and policies.
  • Collaborate with healthcare team members to develop and implement individualized care plans.
  • Communicate effectively with patients, families, and healthcare providers to ensure seamless care transitions.
  • Participate in quality improvement initiatives to enhance patient outcomes and satisfaction.
  • Maintain accurate and detailed records of patient care and progress.
Requirements
  • Graduation from a Licensed Practical Nursing Program approved by the State Board of Nursing.
  • Current licensure in the state where the facility is located or a multistate license issued by a participating NLC state.
  • Ability to work independently and as part of a team to achieve patient-centered care.
  • Strong communication and interpersonal skills.
  • Basic computer skills to utilize the electronic health record.
