We are seeking a highly skilled MDS Nurse to join our team at Solaris HealthCare Apopka. As a key member of our interdisciplinary team, you will play a vital role in ensuring our residents receive the highest quality care.
Key Responsibilities- Conduct thorough and accurate MDS assessments to inform care planning
- Collaborate with the IDT to develop and implement individualized care plans
- Ensure accurate and timely completion of RAI and CAA documents
- Provide education and support to non-licensed staff on assessment and care planning processes
- Participate in ongoing education and training to stay current on MDS 3.0 best practices
- Graduate from an approved school of nursing
- Current, unencumbered license to practice nursing in Florida
- Minimum of two years of RAI and care planning experience in a Skilled Nursing Facility
- Knowledge of MDS 3.0, including assessments, CAA's, and care plans
