SUMMARY:
This role is primarily focused on overseeing patient care, enhancing educational initiatives, supporting self-management, and ensuring timely healthcare delivery through the execution of the following responsibilities.
ESSENTIAL DUTIES AND RESPONSIBILITIES:
Key responsibilities include, but are not limited to:
1. Act as a liaison among patients, families, physicians, and other healthcare professionals regarding healthcare matters.
2. Maintain up-to-date knowledge of OSHA and state regulations, regularly monitoring compliance within the community related to patient care.
3. Conduct follow-up communications with patients to ensure adherence to healthcare recommendations.4. Educate and engage patients to enhance their health outcomes.
5. Assist patients in establishing goals, tracking progress, and identifying obstacles when treatment objectives are not met.
6. Manage various aspects of patient care, including referrals, hospital admissions, emergency room visits, ancillary testing, and other directives to achieve optimal outcomes in a safe and cost-effective manner.
7. Compile a comprehensive list of medical supplies and community resources accessible to patients.8. Operate departmental functions within established budgetary constraints.
9. Coordinate continuity of care with patients and their families to ensure compliance.
10. Provide clinical follow-up with patients, offering information and guidance to enhance communication within the patient-care team.
11. Maintain precise and timely documentation.
SUPERVISORY RESPONSIBILITIES:
This position does not include supervisory responsibilities.
QUALIFICATIONS:

The qualifications listed below represent the knowledge, skills, and/or abilities required. EDUCATION AND/OR EXPERIENCE:
Bachelor's degree (B.A.) from a four-year college or university; or one to two years of relevant experience and/or training; or an equivalent combination of education and experience.
