Position Title: Licensed Practical Nurse (LPN)

Shift: 7PM - 7AM (Part-time)

Compensation: $32 - $36 per hour (includes a $2/hour shift differential for night shifts)

Incentives: $2,000 Sign-on/Retention Bonus

Position Overview:

This role is essential in delivering licensed nursing services to residents, adhering to the protocols established by The Seasons. The LPN will report directly to the Director of Nursing/Health and Wellness Director and may oversee other staff members.

Key Responsibilities:

  • Administer medications to residents in accordance with Federal & State regulations and The Seasons' guidelines.
  • Facilitate the admission process for new residents, including all necessary assessments and documentation.
  • Develop and implement acute and long-term care plans or service plans.
  • Assist in clinical system tracking (e.g., wounds or infection control) under the supervision of the director.
  • Provide general health consultations to residents within the community.
  • Inform physicians and/or family members of any changes in residents' health, ensuring proper documentation is maintained.
  • Respond promptly to all emergencies.
  • Maintain effective communication with all departments and staff members.
  • Conduct educational meetings for residents and staff.
  • Understand the role in the Safety & Disaster Plan.
  • Ensure confidentiality of residents' and prospective residents' information.
  • Participate in all required training, in-service, and staff meetings.
  • Strive to uphold a safe working environment through accident prevention, equipment preservation, and adherence to safe working practices.
  • Exhibit a positive and professional demeanor towards residents, visitors, families, and colleagues.
  • May be responsible for discharges, assessments, and clinical documentation as necessary.
  • Adhere to all policies and procedures of The Seasons.
  • Perform additional duties as assigned.

Qualifications/Skills/Educational Requirements:

Education: Completion of an accredited licensed or vocational nursing program.

Field of Study: General nursing.

Experience: Minimum of 1 year in senior care or a related field.

Certifications: Must possess a current Licensed Practical Nurse's License in the practicing state and be CPR Certified.
