Key Responsibilities:
  • Provide ongoing assessment of patient care needs, using the nursing process to develop and implement individualized care plans.
  • Collaborate with patients and families to develop health care goals and provide education on disease management and self-care.
  • Coordinate with the healthcare team to ensure seamless care transitions and optimize patient outcomes.
  • Participate in quality improvement initiatives to enhance patient care and safety.
  • Stay up-to-date with the latest evidence-based practices and incorporate research findings into nursing practice.
Requirements:
  • Current Registered Nurse License (Washington) or Compact License.
  • Basic Life Support certification.
  • Effective communication, problem-solving, leadership, and decision-making skills.
Preferred Qualifications:
  • Ambulatory care nursing experience.
  • Primary nursing or case management experience.
  • Bachelor of Science in Nursing (BSN).
