Overview:
As a vital member of our healthcare team, the Licensed Practical Nurse (LPN) plays a crucial role in delivering quality care to clients. Under the guidance of a Registered Nurse, you will provide essential support in monitoring patient health, assisting in treatments, and communicating effectively with clients and their families.
Key Responsibilities:
- Effectively communicate health and nursing needs to the supervising RN.
- Facilitate admissions, discharges, and follow-up care as necessary.
- Assist clients during medical appointments and ensure their comfort.
- Deliver nursing services as directed by the RN and prescribed by the physician, adhering to the Service Plan.
- Monitor and document vital signs, identifying any concerning trends.
- Administer medications accurately, following the six rights of medication administration.
- Collaborate with pharmacy staff regarding psychotropic medications and their management.
- Maintain accurate Medication Administration Records (MARs) for each client.
- Ensure the availability of necessary medical supplies and equipment.
- Report any signs of illness or potential health risks to the appropriate healthcare professionals.
- Document all care and services provided in compliance with company and state regulations.
- Provide guidance and education to non-licensed staff regarding patient care.
- Participate in quality improvement initiatives and interdisciplinary team meetings.
Qualifications:
Education: Completion of a state-approved training program (e.g., diploma, associate's degree).
Requirements:
- Current licensure as a Licensed Practical/Vocational Nurse (LPN or LVN).
- CPR certification is mandatory.
- A minimum of 6 months of direct nursing experience is preferred.
- Experience with individuals with developmental disabilities is advantageous.
- Valid driver's license with a satisfactory driving record.
- Proficient in using computer systems for documentation.
- Strong communication skills, both verbal and written.
