Job Summary

As a key member of our care coordination team, you will play a vital role in providing ongoing, extended case management to our patients. Your expertise will be instrumental in coordinating care and referrals to wrap-around services that support clinical care, including enrollment into insurance and benefit programs, transportation, housing, food, behavioral health specialty care, and substance abuse treatment.

Key Responsibilities
  • Work with crisis intervention, complex and chronic mental illness, and substance abuse in a multi-disciplinary team setting.
  • Collaborate with underserved and/or transient populations to provide comprehensive care.
  • Develop and implement care plans that address the unique needs of patients with co-occurring mental health, substance abuse, and physical health problems.
  • Utilize knowledge of general medical practice models and processes to ensure seamless care coordination.
  • Collect and analyze clinical data to inform program development and improve patient outcomes.
  • Effectively communicate with healthcare professionals, community organizations, and patients to ensure high-quality care.
  • Manage multiple tasks and priorities to ensure timely completion of assignments.
Requirements
  • Masters in Social Work or related field preferred.
  • Bachelor's degree in psychology, social work, counseling, or related field, or equivalent education, training, and experience in the behavioral science field.
  • Strong knowledge of HIPAA confidentiality requirements and ability to maintain patient confidentiality.
  • Excellent communication and interpersonal skills, with the ability to engage with diverse patient populations.
  • Proficiency in Word Processing software and database programs, including Excel, and ability to enter information within the Electronic Medical Record.
