Medical Professional for Senior Healthcare Services

We are seeking a compassionate and skilled medical professional to join our team as an Adult – Gerontology Nurse Practitioner at Abode Care Partners. In this role, you will provide high-quality, evidence-based medical care to seniors and patients with complex needs in various settings.
Job Summary:
- Examine and treat chronic and urgent episodic illnesses, minor injuries, accidents, and other injuries or illnesses
- Order and interpret laboratory and diagnostic testing as needed
- Collaborate with caregivers and other healthcare professionals in providing interdisciplinary care and maintaining a comprehensive plan of care
- Provide referrals to other healthcare practitioners when necessary
- Ensure patient medical records are kept up to date
Requirements:
- Active state Nurse Practitioners license
- Current Nurse Practitioner National Board Certification
- Active state DEA licensure
- Master's degree in Nursing (MSN)
- Five or more years of nursing experience; at least two years of nurse practitioner experience preferred
