At Abode Care Partners, we are seeking a skilled Nurse Practitioner to provide evidence-based medical care for seniors and patients with complex needs.
About the RoleThis challenging yet rewarding position involves examining and treating various health issues, collaborating with healthcare professionals, and ensuring the accuracy of patient medical records.

Responsibilities- Perform comprehensive medical assessments and develop individualized plans of care for patients with complex needs
- Provide evidence-based medical treatment for chronic and urgent episodic illnesses, minor injuries, accidents, and other health issues
- Collaborate with caregivers and other healthcare professionals to ensure seamless interdisciplinary care
- Maintain accurate and up-to-date patient medical records
- Active state Nurse Practitioners license
- Current Nurse Practitioner National Board Certification
- Active state DEA licensure
- Master's degree in Nursing (MSN)
- Five or more years of nursing experience; at least two years of nurse practitioner experience preferred
- Strong interpersonal communication skills – verbal and written
- Excellent time management skills with accountability for practitioner's daily workflow
- Proficiency in medical instruments and equipment

Salary RangeThe estimated salary range for this role is $90,000.00 - $108,000.00 per year, based on location and experience.
