About the Role

We are seeking a highly skilled and compassionate Social Services Assistant to join our team at The Grove Elmhurst. As a Social Services Assistant, you will play a vital role in ensuring our residents receive the highest standard of care and support during their stay with us.

Key Responsibilities
  • Prepare residents for their future by coordinating discharge planning and participating in resident care conferences.
  • Ensure residents' social and emotional needs are met by providing support and guidance.
  • Collaborate with the care team to develop and implement individualized care plans that meet the unique needs of each resident.
  • Participate in quality improvement initiatives to enhance the overall care experience for our residents.
Requirements
  • Bachelor's degree in Social Work or a related field.
  • At least two years of experience in a Social Worker role, preferably in a healthcare setting.
  • Prior experience as a Social Services Assistant in a Skilled Nursing Facility is highly desirable.
  • Graduate degree in Social Work or a related field and five years of related experience is preferred.
