About the Job

This position offers a competitive salary of $65,000 - $85,000 per year, depending on experience.

The Clinical Care Manager will work with members who have complex medical needs, providing personalized care plans and supporting their overall health and well-being.

Responsibilities include:

  • Serving as primary care manager for members with complex medical needs
  • Conducting comprehensive needs assessments and developing person-centered care plans
  • Partnering with internal care teams, providers, and community resources to implement care plans
  • Maintaining accurate records and documenting all care management activities

Required qualifications include:

  • A current, unrestricted license as a Registered Nurse
  • 4+ years of relevant clinical work experience
  • 3+ years of experience managing complex populations (e.g., Medicare, Medicaid)
  • 1+ years of community case management experience coordinating care for individuals with complex needs

Preferred qualifications include a master's degree in nursing or a related field, experience working with behavioral health issues, and bilingual skills.
