Job Summary

We are seeking a compassionate Registered Nurse to join our HomeCare Laurens team as a Home Health RN Specialist. This role offers a unique opportunity to provide personalized care to patients in the comfort of their own homes.

About the Position

As a Home Health RN Specialist, you will be responsible for delivering high-quality patient care, conducting thorough assessments, and developing individualized care plans. You will work closely with the interdisciplinary team to ensure seamless delivery of care services.

Responsibilities

  • Assess patients' physical, psychosocial, and environmental factors to identify health risks and develop targeted interventions
  • Collaborate with healthcare teams to coordinate care services and ensure patient satisfaction
  • Provide education and support to patients and their caregivers on various aspects of health care and disease management
  • Maintain accurate records of patient interactions and progress

Requirements

  • Current Registered Nurse license in good standing
  • Minimum one (1) year experience as a RN / professional nurse
  • Experience in home care is preferred

Compensation and Benefits

The estimated annual salary for this position is $70,000 - $90,000, depending on experience. NHC also offers a comprehensive benefits package, including insurance, 401K match, dental, vision, and a dynamic work environment.
