Job Summary

We are seeking a highly skilled Care Manager to join our team at Mindoula Health. As a Care Manager, you will play a critical role in our Teen Suicide Prevention Program, working with at-risk adolescents and young adults to provide support and guidance.

Key Responsibilities:

  • Develop and implement individualized service plans to address the unique needs of each member
  • Provide crisis intervention and support to members in need
  • Collaborate with community resources and healthcare providers to ensure seamless care coordination
  • Conduct regular outreach and engagement with members to assess their needs and provide ongoing support

Requirements:

  • Bachelor's degree in psychology, social work, or a related field
  • 2+ years of experience in behavioral health or a related field
  • Knowledge of local resources and social determinants of health
  • Experience working with at-risk adolescents and young adults
