We are seeking a skilled Behavioral Health Support Specialist to join our team at High Country Behavioral Health. As a Mental Health Care Coordinator, you will play a vital role in providing support and care to individuals with serious and persistent mental illness.
Key Responsibilities- Assist clients with daily living activities, such as bathing, grooming, and dressing.
- Document medication intake and ensure clients are taking their medications as prescribed.
- Assist with shopping and meal planning to ensure clients have access to healthy food options.
- Provide transportation to appointments and community activities.
- Monitor clients for any changes in behavior or medical conditions and report any concerns to medical staff.
- Engage clients in therapeutic activities, such as art, music, and exercise.
- Help clients develop and maintain daily routines and schedules to promote stability and well-being.
- Maintain confidentiality and adhere to HIPAA regulations at all times.
- High School Diploma or Equivalent.
- Previous experience working with individuals in behavioral health preferred.
- Ability to work independently and as part of a team.
- Ability to maintain confidentiality, professionalism, and customer service in all interactions.
- Excellent communication skills and ability to establish rapport with clients.
- Successful completion of HCBH pre-employment screening and background check.
