Job Summary

We are seeking a skilled Home Care Registered Nurse to join our team at Trinity Health Corporation. As a Home Care RN, you will provide primary nursing care to patients in their homes, working collaboratively with our team to deliver exceptional care and achieve top decile client outcomes.

Key Responsibilities
  • Assessment and Care Planning: Accurately assess patient condition and risk at admission, at required time points, and as needed, utilizing appropriate standardized documentation. Develop assessment-based plans of care in collaboration with physicians, Population Health Case Managers, and patients.
  • Collaboration and Communication: Maintain open communication with patients, families, and healthcare teams to ensure seamless care coordination and timely reporting of changes in patient condition.
  • Education and Training: Utilize and implement appropriate educational tools and pathways per guidelines and/or policy and procedure to promote patient education and empowerment.
  • Regulatory Compliance: Maintain a current working knowledge of applicable Federal, State, and local laws and regulations, the Organizational Integrity Program, Code of Ethics, as well as agency policies and procedures and follows Code of Conduct.
Requirements
  • Education: Graduate of an approved nursing education program, BSN preferred.
  • Licensure: Licensure as a Registered Nurse in the State of Connecticut.
  • Experience: One (1) year experience as a professional acute care nurse, with home care experience preferred but not required.
  • Other: Must have current Driver's license and reliable transportation.
