Job SummaryWe are seeking a skilled Registered Nurse to join our Home Health team. As a RN, you will provide compassionate care to patients in the comfort of their own homes, working closely with physicians and other healthcare professionals to deliver individualized care.
Key Responsibilities- Provide professional nursing care to patients in a primary care setting, adhering to nursing practices, agency policies, and regulations.
- Assess patient needs, develop individualized care plans, and implement interventions to achieve optimal patient outcomes.
- Collaborate with the healthcare team to evaluate and improve patient care, ensuring cost-effectiveness and quality.
- Communicate effectively with patients, families, and healthcare professionals to ensure seamless care coordination.
- Document patient care and services provided, adhering to Duke Health's policy and procedures.
- Associate Degree or Diploma in Nursing required.
- 1-2 years of med-surgical, critical care, or home health/hospice/infusion experience preferred.
- Prior experience in Home Health, Hospice, or Infusion preferred.
- Prior OASIS and PDGM experience preferred for Home Health.
