About the Role

We are seeking a compassionate and skilled Registered Nurse to join our team at Duke Health, providing exceptional care to patients in their homes and communities. As a Registered Nurse, you will be responsible for delivering high-quality, patient-centered care, working collaboratively with our interdisciplinary team to ensure the best possible outcomes for our patients.

Key Responsibilities
  1. Assess patient needs and develop individualized care plans to meet their unique needs and goals.
  2. Provide direct patient care, including administering medications, wound care, and other treatments as prescribed by physicians.
  3. Collaborate with patients, families, and healthcare teams to ensure seamless care transitions and optimal patient outcomes.
  4. Document patient progress, including assessments, interventions, and outcomes, to ensure accurate and timely communication with the healthcare team.
  5. Participate in ongoing education and professional development to stay current with best practices and evidence-based care.
Requirements
  • Current RN licensure in North Carolina or compact state.
  • 2+ years of experience in home health, hospice, or infusion nursing.
  • Valid driver's license and reliable transportation.
  • Ability to work autonomously and collaboratively as part of a team.
