About the Role

Aylo Health is seeking a highly skilled Clinical Care Coordinator to join our team at our McDonough-Kelly Rd office. As a key member of our Care Team, you will play a vital role in managing patient encounters and clinical relationships, ensuring a best-in-class patient experience.

Key Responsibilities
  • Initiate outgoing calls to patients with pending post-visit care to ensure timely and accurate processing of diagnostic imaging, labs, prescriptions, and other activities.
  • Manage patient care and provider relationships, serving as a point of escalation for Level I Clinical MAs.
  • Process e-prescription requests from pharmacies and submit patient medication as instructed by providers.
  • Correspond with providers through the electronic medical record system (eClinicalWorks) regarding patient activity.
  • Answer patient inquiries regarding general medical questions and obtain necessary personal and clinical information.
  • Accurately document telephone encounters and receive test results from providers.
  • Contact patients by phone in a timely manner to convey test results accurately and privately, following all HIPAA regulations.
Requirements
  • High School graduate or equivalent education.
  • Proven history of driving a positive patient experience.
  • For MAs: Completion of an accredited Medical Assistant Program with a current Medical Assistant Certification/Registration.
  • For LPNs: Completion of an accredited LPN program.
  • 5+ years' work experience in a clinical private practice setting with EMR experience.
  • Ability to work quickly and efficiently to initiate high volumes of outgoing calls.
  • Demonstrated ability to organize, coordinate, prioritize, and facilitate many on-going tasks.
  • Professional verbal and written communication skills.
  • Ability to work independently or with a team.
  • Must be able to report to work as scheduled without delay; to include rotating evening shifts until 7pm.
  • Professional appearance and positive attitude.
  • Excellent communication and problem-solving skills.
