About the Role

We are seeking a highly skilled RN Care Coordinator to join our team at CHSGa. As a key member of our healthcare team, you will play a vital role in ensuring the delivery of high-quality patient care.

Key Responsibilities
  • Facilitate effective communication between nursing center departments and patients/family members to provide comprehensive care plans.
  • Establish and maintain open lines of communication with interdisciplinary team members to ensure seamless care coordination.
  • Assist in the development and implementation of patient care plans, ensuring timely and accurate completion.
  • Monitor and review patient assessments and care plans to ensure compliance with regulatory requirements.
  • Provide education and training to staff members on patient care planning and MDS processes.
  • Collaborate with the Director to ensure the success of patient care initiatives and projects.
Requirements
  • A Nursing Degree from an accredited college or university, or an approved RN program.
  • Active RN License to practice in the State of Georgia.
  • Excellent organizational and prioritizing skills, with the ability to work in a fast-paced environment.
  • Effective and professional verbal and written communication skills.
