About the Role

We are seeking a highly skilled and compassionate Kidney Transplant Coordinator to join our team at Henrico Doctors' Hospital Forest. As a key member of our transplant team, you will play a vital role in ensuring the smooth coordination of kidney transplant services for our patients.

Responsibilities
  • Communicate effectively with physicians, hospital staff, and external consumers to coordinate pre and peri-operative processes.
  • Assess transplant patients for specific needs and educate them and their families regarding their needs related to the kidney transplant, including ongoing follow-up care requirements.
  • Plan follow-up care in collaboration with the referring physician, transplant surgeon, and other care providers.
  • Analyze pertinent data, communicate findings, document information, and assume accountability for decisions and actions.
Requirements
  • Active RN license in the state of Virginia.
  • BSN preferred.
  • At least one year of ICU experience required.
  • Three years of nursing experience with 1 year in ICU and one year case management preferred.
