Job Title: Social Worker III

Job Summary:

The Social Worker III performs and documents highly complex social services work, serving as a liaison between patients, legally authorized representatives, and others to ensure ongoing care, treatment, and support. This position gathers information to assess the individual's support systems and service needs, including participation in interdisciplinary team meetings as needed.

Key Responsibilities:
  • Interviews and converses with individuals, legally authorized representatives, actively involved persons, and agencies to create and maintain the individual's social history.
  • Oversees the communication and information exchange between the individual, legally authorized representative, actively involved person, and agencies, as needed, and facility or treatment providers to ensure ongoing care, treatment, and support.
  • Counsels and provides assistance to individuals, legally authorized representatives, and actively involved persons regarding community placement issues, living options discussions, end-of-life planning, and rights.
Requirements:

To be successful in this role, you will need to have a strong understanding of social services principles and practices, as well as excellent communication and interpersonal skills. You will also need to be able to work effectively in a team environment and have a strong commitment to providing high-quality care and services to individuals and families in need.
