Key Responsibilities
  • Develop and Implement Patient Care Plans

Design and implement individualized plans of care for patients, incorporating participation from significant others when appropriate. Conduct thorough assessments to identify patient needs and develop targeted interventions to maximize progress toward functional goals.

Provide Direct Patient Care

Deliver high-quality, patient-centered care, evaluating functional needs and outcomes, and consulting with other specialists as needed. Review patient progress and adjust care plans to ensure optimal outcomes.

Collaborate with Interdisciplinary Team

Communicate effectively with other healthcare professionals and support staff to achieve positive patient outcomes. Participate in interdisciplinary team meetings to review and update patient care plans.

Document Patient Progress

Accurately document evaluations, treatment goals, and plans via our electronic medical record system. Regularly update documentation and maintain prescriptions and signatures.

Stay Current with Professional Development

Participate in ongoing training and education to maintain current knowledge and skills in physical therapy practice. Stay up-to-date on professional literature and best practices.

Lead and Mentor Team Members

Provide direction and leadership to staff, acting as a resource to promote physical therapy in the community. Collaborate with the agency's management team to maintain quality patient care and achieve agency goals.

Requirements
  • Current licensure as a Physical Therapist (PT), Occupational Therapist (OT), or Speech Therapist (ST)
  • Experience in home health or similar setting
  • Strong communication and interpersonal skills
  • Ability to work independently and as part of a team
  • Commitment to delivering high-quality patient care and achieving exceptional outcomes
