Home Health RN Case Manager

Signature Healthcare at Home is seeking a skilled Home Health Registered Nurse Case Manager to join our team. As a key member of our care team, you will provide high-quality patient care and coordinate services to meet the needs of individuals and families in their homes and communities.
Responsibilities
The successful candidate will be responsible for:
- Completing comprehensive assessments of patients and their living situations to determine care needs.
- Providing professional nursing care by utilizing the nursing process and adhering to state Nurse Practice Act guidelines.
- Establishing individualized plans of care in collaboration with certifying medical providers.
- Regularly re-evaluating patient and family/caregiver needs and revising plans of care as necessary.
- Developing care plans that establish interventions and goals, involving patients and families/caregivers in the planning process.
- Administering medications and treatments as prescribed by medical providers.
- Facilitating patient efforts toward self-sufficiency or obtaining appropriate caregiving.
- Acting as Case Manager when assigned by Clinical Manager and coordinating patient care for assigned caseloads.
Qualifications
The ideal candidate will possess:
- A graduate degree in nursing from an accredited school.
- One to two years of recent acute care experience in an institutional setting, and one to two years of recent experience in home health preferred.
- Current licensure as a Registered Nurse in the State(s) of planned practice.
- Possession of CPR certification.
- A valid driver's license with an automobile that is insured in accordance with state and/or organization requirements.
- Excellent observation, verbal, and written communication skills, problem-solving skills, basic math skills, and nursing skills per competency checklist.
- Self-directed and able to work with minimal supervision.
- Capable of prolonged or considerable walking or standing, lifting, positioning, and/or transferring patients, lifting supplies and equipment, and considerable reaching, stooping, bending, kneeling, and/or crouching.
