Job Summary

We are seeking a skilled and compassionate Registered Nurse Case Manager to join our team at CenterWell Home Health. As a key member of our care team, you will provide high-quality nursing care to patients in their homes, working closely with physicians and other healthcare professionals to develop and implement personalized care plans.

Key Responsibilities
  • Provide skilled nursing visits to home health patients, including admission, case management, and follow-up visits
  • Confer with physicians to develop treatment plans based on patient assessment and physician's orders
  • Coordinate care for patients with various healthcare personnel, and report patient progress to physicians and Clinical Manager
  • Conduct hands-on care, management, and evaluation of care plan, including teaching patients in accordance with physician orders
  • Supervise Home Health Aides and Personal Care Workers, providing written instructions and monitoring documentation
  • Prepare medical documentation, including patient records, case conferences, and care plan changes
Requirements
  • Diploma, Associate, or Bachelor Degree in Nursing
  • Minimum of one year nursing experience preferred, with strong med surg, ICU, ER, or acute care experience
  • Home Health experience is a plus
  • Current and unrestricted Registered Nurse licensure and CPR certification
