We are seeking a skilled Patient Services Coordinator LPN to join our team at CenterWell Home Health. As a key member of our care team, you will play a vital role in ensuring seamless patient care coordination and continuity.

Key Responsibilities:
  • Schedule Management: Manage patient schedules, edit schedules for agents calling in sick, and update agent unavailability in the worker console.
  • Infection Control: Initiate infection control forms as needed and send completed reports to HRD for upload in the worker console.
  • Communication and Support: Serve as a backup during busy times, receive calls from field staff, and assist with weekly case conferences. Refer clinical questions to the Branch Director as necessary.
  • Client Hospitalization Log: Maintain the client hospitalization log, including entering coordination notes and sending electronic logs to all office, field, and sales staff.
  • Schedule Completion: Complete requested schedules as tasks appear on the action screen, ensuring staff are scheduled for skilled nurse/injection visits unless an aide supervisory visit is scheduled in conjunction.
  • Add-on Scheduling: Complete requested schedules for all add-ons and applicable orders, including scheduling discharge visits/OASIS collection or recert visits, rescheduling declined or missed visits, and processing reassigned and rescheduled visits.
  • Reporting and Scheduling: Run all scheduling reports, including the Agent Summary Report and Missed Visits Done on Paper Report, and prepare weekly agent schedules.
  • Verification and Assistance: Verify visit paper notes in the scheduling console as needed and assist with internal transfers of patients between branch offices.
  • Clinical Support: If clinical, receive lab reports, assess for normality, fax copies to doctors, make copies for Case Managers, and route to Medical Records Department. Initiate Employee/Patient Infection Reports as necessary.
  • On-Call Rotation: If clinical, may be required to perform patient visits and/or participate in on-call rotation.
Requirements:
  • Licensure: Be a Licensed Professional Nurse or a Licensed Vocational Nurse licensed in the state in which you practice.
  • Experience: Have at least 1 year of home health experience.
  • Preferred Qualifications: Prior packet review/QI experience and coding certification.
  • Additional Requirements: Possess a valid state driver's license, automobile liability insurance, and be currently licensed in the State of employment if applicable.
  • Essential Skills: Excellent communication skills, ability to interact well with diverse individuals, strong organizational skills, and ability to manage and prioritize multiple assignments.
