We are seeking a highly skilled and experienced Nursing Manager to lead our Hospital at Home program. As a key member of our team, you will be responsible for managing staff, overseeing patient care, and ensuring adherence to established policies and procedures.

Key Responsibilities
  • Lead and manage a team of nurses in the Hospital at Home program
  • Develop and implement strategies to improve patient outcomes and satisfaction
  • Collaborate with interdisciplinary teams to ensure seamless care transitions
  • Monitor and evaluate program performance, identifying areas for improvement
  • Develop and maintain relationships with community partners and stakeholders
Requirements
  • Bachelor's degree in Nursing
  • 5 years of clinical experience in a hospital or home health setting
  • Current RN licensure and Basic Life Support certification
  • Strong leadership and communication skills
  • Ability to work in a fast-paced environment and prioritize multiple tasks
