About the Role:

  • Coordinate referrals and connect patients with community resources and services.
  • Assist patients with accessing medical care, including transportation and interpreter services.
  • Participate in ongoing education and training to enhance knowledge and skills.

Requirements:

  • High school diploma or equivalent required; Associate's or Bachelor's degree preferred.
  • At least two years of experience in a healthcare setting, preferably in a patient navigation or social work role.
  • Strong understanding of healthcare systems and policies.
  • Bilingual skills a plus.
